How To Request Face Shields

Fill out our very simple google form and print/sign this waiver, and we will start working on your 3D printed face shield order.  Once you submit your request, you will receive a confirmation email. We strive to deliver your requests in 1-2 weeks, but it depends on how many orders we have at any given time.  Please note that we are not able to deliver face shields unless the waiver is signed and emailed back to huangview@gmail.com.
